Introduction
Welcome to the website  of Tele-Evaluations 360 PLLC ("The Practice"). These Terms and Conditions ("Terms") govern your use of our website and the services provided by The Practice. By accessing or using our website, you agree to comply with and be bound by these Terms. If you do not agree with these Terms, you are not authorized to use our website or services.


1. Services Provided
The Practice provides forensic mental health evaluations for court matters, with a specialty in trauma. A forensic mental health evaluation is a comprehensive assessment conducted by a qualified mental health professional to provide an expert opinion for legal proceedings. These evaluations typically consist of:

  • Clinical Interviews: In-depth conversations with the individual to gather detailed information about their psychological history, current mental state, and relevant experiences.

  • Psychological Testing: Standardized tests and assessments to measure various aspects of the individual's mental health, cognitive abilities, and personality traits.

  • Collateral Information: Review of additional information such as medical records, legal documents, and interviews with family members or other relevant parties.

  • Report Preparation: A detailed written report that summarizes the findings of the evaluation, provides a professional opinion, and may include recommendations for treatment or further assessment.

 

These evaluations are conducted with the utmost care and confidentiality, ensuring that the information gathered is used appropriately to support the legal process. Please note that we do not provide court testimony.


2. Use of Website
Eligibility: You must be at least 18 years old to use our website and services.
Payment Requirement: To engage in our services, you must provide a deposit payment of $150.00 over the phone first. After payment, you will receive a HIPAA-compliant link via text to enter your personal information and complete the registration process.


3. Privacy Policy
Our Privacy Policy, which outlines how we collect, use, and protect your personal information, is incorporated into these Terms by reference. By using our website, you agree to the terms of our Privacy Policy.


4. Payment and Refunds
Fees: The current fee schedule is as follows (subject to change):
Regular: $950.00
Express: $1.200.00

 

  • Payment: A deposit of $150.00 is required at the time of service booking. The balance will be charged on the day of the appointment, at the scheduled appointment time. The client consents to this automatic charge. Payments must be made via debit or credit card. We do not accept insurance or offer payment plans.

  • Refund Policy: No refunds or partial refunds are offered regardless of the outcome of the forensic mental health evaluation, or if it is no longer needed.


5. Forensic Mental Health Evaluations process

  • Appointment Scheduling: You must provide at least 48 hours' notice to cancel or reschedule your appointment. Failure to provide 48 hours' notice or arriving more than 15 minutes late will result in the forfeiture of the deposit ($150.00).

  • Adhering to the Registration Process: You must complete the registration process shared with you at the time of the reservation at least 48 hours' before your scheduled appointment. Failure to do so will result in the forfeiture of the deposit ($150.00).

  • Conduct During Evaluation: The forensic mental health evaluation is conducted via video call. You must be alone during the session to ensure privacy and confidentiality. Ensure you have a stable internet connection, a working camera, and a microphone for the video call.

  • Post-Evaluation: The forensic mental health evaluation will be sent via email to the elected party within 2 weeks for the Regular Process and within 72 hours of the appointment time for the Express Process.

6. Disclaimer of Warranties and Limitation of Liability

  • No Warranties: The Practice makes no representations or warranties about the suitability, reliability, availability, timeliness, and accuracy of the information, software, products, services, and related graphics contained on the website for any purpose. All such information, software, products, services, and related graphics are provided "as is" without warranty of any kind.

  • Limitation of Liability: In no event shall The Practice be liable for any direct, indirect, punitive, incidental, special, consequential damages, or any damages whatsoever including, without limitation, damages for loss of use, data, or profits, arising out of or in any way connected with the use or performance of the website, with the delay or inability to use the website or related services, the provision of or failure to provide services, or for any information, software, products, services, and related graphics obtained through the website, or otherwise arising out of the use of the website, whether based on contract, tort, negligence, strict liability, or otherwise, even if The Practice has been advised of the possibility of damages. If you are dissatisfied with any portion of the website, or with any of these terms of use, your sole and exclusive remedy is to discontinue using the website.


7. Governing Law
These Terms and all matters arising out of or relating to these Terms shall be governed by and construed in accordance with the internal laws of the state in which The Practice operates without giving effect to any choice or conflict of law provision or rule.


8. Changes to Terms
The Practice reserves the right to change these Terms from time to time. We will notify you about significant changes in the way we treat personal information by sending a notice to the primary email address specified in your account, by placing a prominent notice on our website, and/or by updating any information on this page. Your continued use of the website and/or services available through this website after such modifications will constitute your acknowledgment of the modified Terms and agreement to abide and be bound by those Terms.

10. Emergency Procedures
In case of an emergency, please contact crisis hotlines or emergency services immediately. If you are in immediate danger, call 911.


11. Client Responsibilities
Provide Accurate Information: Clients must provide accurate and complete information about their health and treatment history.
Adherence to RegistrationProtocol: Clients must adhere to the registration at least 48 hours before their scheduled forensic mental health evaluation, otherwise, their deposit of $150.00 will be forfeited.


12. Intellectual Property
All content included on this site, such as text, graphics, logos, images, audio clips, digital downloads, data compilations, and software, is the property of The Practice or its content suppliers and protected by United States and international copyright laws.


13. Feedback and Dispute Resolution

  • Client Feedback: Clients are encouraged to provide feedback via surveys or suggestion boxes. Client feedback is used to improve services and address identified issues.

  • Dispute Resolution: Any disputes regarding billing or services must be submitted in writing within 30 days of the charge. If services are terminated due to reasons such as verbal abuse or any form of abuse or extortion, the client's investment will be forfeited.


14. Additional Consent and Confidentiality
You consent, understand, and agree that:

  • You have the right to discuss the risks and benefits of all assessments and referrals for treatment provided by The Practice's providers along with any available alternatives.

  • The Practice will provide care in accordance to the standards of behavioral health practice but does not guarantee or assure treatment outcomes.

  • You have the right to review and receive copies of your mental health records, including all information obtained during a telehealth interaction, subject to The Practice's standard policies regarding the request and receipt of behavioral health records and applicable law.
